TEHRAN, Iran — Iranian President Masoud Pezeshkian was reportedly injured in last month’s Israeli airstrikes targeting a secret underground facility in Tehran, according to a report by Iran’s Fars news agency, which is closely linked to the country’s Islamic Revolutionary Guard Corps (IRGC).
The strike allegedly occurred on Monday, June 16, 2025, during the fourth day of the 12-day war between Israel and Iran.
Six precision-guided bombs were said to have targeted all access and ventilation points of the underground bunker, where Pezeshkian was attending an emergency session of Iran’s Supreme National Security Council.
Fars reports that President Pezeshkian sustained minor leg injuries while escaping through an emergency shaft along with other senior Iranian officials.
The underground facility reportedly lost power and air circulation following the strike.
The report, which has not been independently verified, raises renewed questions over how Israel acquired intelligence on the location and movements of Iran’s top leadership.
The Israeli Defence Forces (IDF) have not publicly commented on the matter.
Social media footage from mid-June appeared to show explosions near a mountainside in north-western Tehran, but it was not previously known that the site housed one of Iran’s most secure strategic facilities.
The Supreme National Security Council is Iran’s highest decision-making body after Supreme Leader Ayatollah Ali Khamenei.
Sources within the Iranian government now suggest an ongoing internal probe is investigating possible infiltration by Israeli operatives or foreign intelligence assets, given the surgical accuracy of the strikes.
President Pezeshkian, who has kept a low profile since the end of the hostilities, last week accused Israel of attempting to assassinate him.
“They tried to kill me,” he reportedly told a closed-door session of parliament, according to Iranian media outlets.
Israel’s Defence Minister, Israel Katz, denied the claim, stating that “regime change was never a goal of the military operation.”
However, Israeli officials previously admitted that Ayatollah Khamenei was also a target during the early phase of the war.
According to security sources, the Iranian Supreme Leader was moved to a highly classified location, effectively cutting off communication with much of the government and contributing to what some insiders have called “a 24-hour command paralysis.”
On Friday, June 13, 2025, Israel launched a surprise wave of air and cyber attacks on Iranian nuclear and military sites, stating it acted to halt Iran’s alleged progress toward building nuclear weapons.
Iran retaliated with aerial drone and missile attacks on Israeli cities and military bases. Tensions further escalated when, on Sunday, June 22, 2025, US forces carried out coordinated strikes on three of Iran’s nuclear facilities.
President Donald Trump later claimed the operation had “obliterated” the sites, though US intelligence has been more reserved in its assessment.
Tehran insists its nuclear programme is for peaceful energy development and medical research.
However, the extent of Israel’s targeted campaign — including the elimination of several top IRGC and army commanders in the early hours of the conflict — has intensified international scrutiny of the scale and precision of Israeli intelligence operations within Iranian territory.
The Iranian government has yet to officially confirm President Pezeshkian’s injuries.
